Required Athletic Forms

The anticipation of each athletic season brings about an increased level of excitement for both parents and athletes alike. However, in order to participate with an athletic team, New Milford requires certain forms to be completed.

The New Milford athletic department combined all required forms into a form labeled SPORTS PERMISSION PACKET. Prior to each season student/athletes and parents are required to complete and submit this form.
